Anne Maher RD, LDN CNSC is the home infusion dietitian at Baystate Home Infusion & Respiratory Services. Anne’s responsible for managing nutrition support for hundreds of patients at a time who are at various stages during their life cycle and encompassing all different kinds of disease states. She coordinates daily with inpatient and long term care dietitians, doctors, insurance companies and pharmacists to ensure optimal continuity of care for her patients while they transition from the inpatient to home or long term care setting.

Anne will share her expertise and discuss characteristics and considerations of long term parenteral nutrition (PN) for patients at home or in a skilled nursing facility.

Objectives:
 Identify current indications for home and long term parenteral nutrition use.
 Understand formulation considerations for home and long term PN.
 Evaluate components of home and long term PN monitoring and management.
 Understand reimbursement issues for parenteral nutrition.
 Discuss criteria for a safe transition of the patient receiving PN from one healthcare setting to another, or to home.
